- Name, phone number, and e-mail address of designated Service Administrator
- Number of locations and offices
- Phone system maintenance vendor’s name, address, and phone number
- # of lines for each office location
- Exact emergency location address of each office for 911 response & signed acknowledgement of 911 notification
- Phone number types necessary
- Model and name of existing phone equipment
- Preference for billing (monthly invoice or credit card)
- Detailed list of existing local and toll-free telephone numbers that you wish to use with new service
